Your Right to Privacy:
Renaissance Plastic Surgery collect personal and health information about you, we are committed to the protection of this personal information provided to us. Mr. Brown and his experienced staff of professionals are dedicated to delivering the highest surgical outcome possible to our patients. The privacy, safety and satisfaction of all patients are our priority. Our practice has developed a policy to protect patient privacy in compliance with privacy legislation (the National Privacy Principles of the Commonwealth Privacy Act 1988).
These acts provide you with a number of rights including:
• The right to be informed of why your personal and health information is being collected and whether it can be given to any other parties; • Your personal and health information can only be collected for lawful purposes; • You have the right to request to see what information is held about you and have it corrected if it is incorrect, out of date or incomplete; • The right to have your personal and health information stored securely and protected from unauthorized access or misuse; • The right to know how your personal and health information is being used; • The right to complain to Mr Brown/Renaissance Plastic Surgery if you believe that your privacy has been violated
What Personal and health information about you is being collected:
• Your name • Your contact details, including address and telephone number/s • Your date of birth • Occupation • Your referring doctor and/or you General Practitioner • Your private health fund details if applicable • Your Medicare/Veterans Affairs number • Results of any tests or procedures • Information regarding past medical history • Name of next of kin • Any allergies you may have • Any medication you may be taking • Pre and post operative clinical photographs
What Mr Brown and his staff do with your health and personal information:
Personal and health information is used for:
• The provision of services and quality care for you • For internal administrative requirements • To assist staff in providing information and care to you • Assist Mr Brown and his staff when you make phone telephone calls to the rooms • To provide necessary information to hospitals and their staff where appropriate • To provide necessary information to anesthetists where appropriate • To keep appropriate documentation in accordance with legal requirements
Where your health and personal information may be disclosed to a third party without your consent:
• An emergency situation • By law, occasionally doctors are required to disclose information for public reasons, for example mandatory reporting of communicable diseases. • Provide Medicare or private health funds if appropriate for the purposes of rebates • Your involvement in unlawful activity • Debt collection agencies in the event of default on payment following fair warning
Patient photographs:
Patient photographs are an important part of the cosmetic plastic surgery record. It assists Mr. Brown to deliver the highest possible results. These photographs are only able to be accessed by Mr. Brown or his professional staff.
Those patients who have provided us with consent to use their before and after photographs, in the interests of educating other patients considering cosmetic plastic surgery have done so in writing. Only photographs of patients who have given their specific written consent are used, this is without exception.
Where your information is stored:
Your personal and health information is stored within our software programme and in patient files. Patient files are kept strictly confidential and are stored away from public access areas. Computer generated patient details are not sent to any other party without your consent.
It is a legal requirement that we store your records for a period of time after your last attendance, these will be archived and stored within a secure and private and safe area.
Internet Site Disclaimer
We do not collect personal information about you when you visit our web site unless you choose to provide this information to us via email contact direct from the site. However, certain information about site visitors is automatically collected and stored.

What to do if you believe your privacy has been breached?
If you believe Renaissance Plastic Surgery has breached your privacy in any way you have the right to complain. The complaint must be in writing and directed to the Principals of Renaissance Plastic Surgery who will conduct an internal review immediately and report back to you with their findings, in writing.
If you are dissatisfied with Renaissance Plastic Surgery findings you have the right to complain to the Federal Privacy Commissioner.
